OUT OF THE AREA AND LOCAL COMMAND:
To scheduled a patient you must follow this procedure:


  1. The POC will call Scheduling Section at (619) 553-0373 or 553-0370. The Staff at Scheduling Office will initiate the Request of Service Form.



  1. Faxed a copy of the LIP where the Medical Doctor put the DX and what level of treatment of the patient (SF600) ASAP. Fax number 619 553-9448




  1. Faxed a copy of the completed SHAPE FORM ASAP. This evaluation must be completed 30 days prior to SARD TX date. (1st page completed by patient and 2nd page completed by his physician).




  1. The patient Doctor and SARD Doctor confirmation is important before patient reporting to SARD Facility. (OUT OF THE AREA)




  1. After the entire requirement above is received and approved, will call you of the bed date confirmation.




  1. Patient must bring at least 5 weeks supply of any maintenance

Medication (i.e. medicine for high blood pressure etc.) prescribed by his/her

Doctor.


  1. Patient must be sober for at least 5 days prior to treatment.




  1. Patient must bring service, medical and dental records.




  1. Patient must be in uniform of the day when reporting for treatment.


NOTE: SCREENING AND LIP MUST BE WITHIN 6 MOS BEFORE TX DATE.

POINT LOMA CLINIC

General Information and Rules and Regulations
A COPY OF THIS WELCOME ABOARD MUST BE GIVEN TO THE PATIENT TO READ BEFORE REPORTING FOR TREATMENT.
1. Introduction. Welcome to the Point Loma Clinic of the Substance Abuse Rehabilitation Dept (SARD), Naval Medical Center, San Diego.
Patients must be sober for 5 days prior to treatment. Prior to arriving for treatment, you should have been informed by your DAPA/SACO/CDAR of the particular treatment program to which you have been assigned. If you have not been briefed on your treatment program, please contact your command Drug and Alcohol Representative.
Based on your individual treatment needs, you were assigned to one of three different treatment programs offered by SARD. In general, these different programs are:


  1. Outpatient Half Day: A two-week program conducted Monday through Friday for half-day (0745 – 1200). This program is designed for patients diagnosed as alcohol abusers. Patients will attend treatment in the mornings and return to their parent command in the afternoons. Treatment involves participation in small and large group counseling, workshops and task groups, and self-help/12-Step meetings.

b. Intensive Outpatient: A four-week program conducted Monday through Friday. This program is designed for patients diagnosed as alcohol dependent. During the first two weeks (Phase I), you will attend treatment for full days with evening self-help/12-Step meetings required. During weeks three and four (Phase II), you will attend treatment for half-days in the mornings (if you are assigned to a local command, you will be directed to return to your parent command). If patient is from out of the San Diego area and assigned to this treatment program, you will be required to berth in Building 500. Regular or weekend liberty may be granted and patients are required to stay in the berthing and return by 2300 daily. Treatment consists of participation in small and large group counseling, workshops and task groups, PT and self-help/12-Step meetings.


c. Partial Hospitalization: A four-week program conducted full days, Monday through Sunday. This program is designed for patients diagnosed as alcohol dependent. If patient is from out of the San Diego area and assigned to this treatment program, you will be required to berth in Building 500. Regular or weekend liberty may be granted and patients are required to stay in the berthing and return by 2300 daily. Treatment consists of participation in small and large group counseling, workshops and task groups, PT and self-help/12-Step meetings.
2. REPORTING FOR TREATMENT
a. Upon reporting for treatment, you must bring:
(1) TDY/TAD Orders (Patient due to be separated upon completion of treatment will report back to their Command.)
(2) Service Record
(3) Outpatient Medical and Dental Record
(4) Copy of your CAAC/SACO/CDAR/SAPM/UADC File in a sealed envelope (Required for all Alcohol and Drug Treatment Programs.)
b. Arrival for Treatment (Transportation Arrangements)
(1) MEDEVAC. If you plan to travel via MEDIVAC, you can expect to arrive here on Saturday or Sunday. As it may take anywhere from two to five days to arrive, and you will not see your luggage until you arrive here, a carry-on overnight bag with a complete change of clothing, night clothes and toiletry items is strongly recommended.
(2) Commercial Air. If you will be traveling via commercial air, you must report to the SARD Quarterdeck no earlier than 0700 Sunday prior to treatment and no later than 0730 on your assigned treatment start date. Upon arriving in San Diego, take a cab/shuttle bus to Submarine Base Point Loma. If you have any questions or problems, contact the SARD Quarterdeck at Commercial (619) 553-0084. All personnel traveling via commercial air must report with round-trip transportation.
(3) Privately-Owned Vehicles/Other Transportation. If you will be arriving via POV from a local command, you must report at 0730 on your assigned treatment start date.
If you will be arriving via POV from out of the area, you must report no earlier than 1600 Sunday prior to treatment start date.
b. Arriving later than required report date and times will result in cancellation of your bed/quota. You will be returned to your command and asked to reschedule treatment.
c. Location. The Point Loma Clinic is located in Building 500 on board the Naval Submarine Base, Point Loma, San Diego, California. To reach Building 500, from either Interstate-5 or Interstate-8 (West), exit at Rosecrans Avenue. Continue south on Rosecrans Avenue through the SUBASE Main Gate past McDonalds. Building 500 is located on the East Side of Rosecrans, directly across the street from a three-story parking garage and the Autoport gas station.
d. Parking. Parking can be found in either of the three-story parking garages located near Building 500. Additional spaces for E-7 and above are available just south of Building 500.

(1) For all patients: please park only in authorized spaces. Those parking illegally may be ticketed and towed. SARD is not responsible for any patient vehicle ticketed or towed.


e. What to bring into Treatment.


(1) Partial Hospitalization Patients and all patients berthing in Building 500: SARD supplies bed linens, but you will need to bring personal items including:
- Towels/washcloths

- Laundry/detergent

- Alarm clock

- Valid Drivers License

- Two padlocks
(2) Intensive Outpatient:

- One combination lock

- PT Gear on identified days (see requirements under uniforms, civilian attire and grooming.)
(3) Outpatient: All material will be provided.
b. What not to bring into Treatment
(1) Electronic Equipment (video games, TVs, laptop computers)
(2) Weapons

(3) Food and Beverages

(4) Drug-related paraphernalia

(5) Pornography

(6) Non-prescription medications; nutritional supplements.




  1. BERTHING AND MESSING

a. Berthing




  1. Berthing facilities are available for patients diagnosed by a Licensed Independent Practitioner or Medical Officer as requiring Partial Hospitalization or Domicillary services. Additionally, berthing is available for patients from outside the San Diego area. Patients from the San Diego area will not normally be berth unless patients’ command will be underway during their treatment.




  1. Each treatment floor provides four-person rooms, a lounge and free laundry facilities.

b. Messing
(1) Patients are strongly encouraged to eat three meals a day. The NAVUBASE Point Loma Galley is located adjacent to Building 500. Meal hours are:
Monday –Friday Saturday, Sunday, Holidays
Breakfast 0630-0730 0730-0830

Lunch 1100-1230 1100-1230

Dinner 1630-1730 1630-1730
(2) A meal pass will be issued to enlisted personnel not currently receiving COMRATS. All other personnel (enlisted drawing COMRATS and officers) will be required to pay for their meals.
(3) Meal prices for active duty military (Officer & Enlisted on COMRATS), Retired, and Dependents will be charged $1.60 for Breakfast, and $3.25 for Lunch and Dinner. Personnel who are issued a Chowpass will not be charged for their meals. Dependents and retirees are reminded that it costs approximately $10.00 a day to eat in the galley.
(4) Personnel drawing COMRATS must bring a copy of their most current LES in order to continue COMRATS.
(5) Coast Guard personnel must be receiving SEPRATS.
(6) For personnel not drawing COMRATS who demonstrate the inability to budget money to pay for their meals, COMRATS will be cancelled and a meal pass issued.
4. UNIFORMS, CIVILIAN ATTIRE AND GROOMING
a. SARD Uniform of the Day Authorized. For all treatment programs, uniform of the day is required (as listed below). Clean working uniforms are also required.
b. Uniform Requirements for all Patients:
The following uniform requirements are the minimum for patients to entire treatment. Active duty personnel under the UCMJ authority must have a complete seabag (Military issued compliment) of uniforms in their possession during TDY status to this facility.


  1. Navy Members: E1 through E6: Three sets of the prescribed Uniform of the Day (Summer Whites/Winter Blues). E7 and above: Are authorized to wear dress khaki. For PT, running shoes and appropriate PT gear.




  1. Marine Corps Members: All ranks: Three sets of the prescribed Uniform of the Day (Uniform Charlie). For PT, running shoes and appropriate PT gear.




  1. Army Members: All Ranks: Three sets of the prescribed Uniform of the Day. (Class B). For PT, running shoes and appropriate PT gear.



  1. Air Force Members: Three sets of Blues in good condition, For PT, running shoes and appropriate PT gear.




  1. Coast Guard Members: Three sets of Undress Blues and 1 set of Tropical Blues-Long. For PT, running shoes and appropriate PT gear.




  1. Retirees and Family Members: Three sets of clean, casual attire. For PT, running shoes and appropriate PT gear.

Note 1: The summer uniform season is from the first Monday in April through the last Sunday in October. The winter uniform season is from the last Monday in October through the first Sunday in April. There is no optional period.


Note 2: Evening and early morning temperatures can drop into the 40's. Appropriate civilian and workout attire (including a jacket and athletic sweats) is recommended.
c. Civilian Clothes
(1) Your participation in community self-help meetings necessitates that you bring appropriate, casual civilian attire sufficient for the duration of your stay. Tank tops, cut-off shorts/shirts, or any clothing with reference to drugs, alcohol, tobacco, sex; profanity or violence will not be permitted.
(2) Military standards of grooming, courtesy and behavior are mandatory at all times. Patients will wear appropriate military/civilian attire at all times. The wearing of earrings by male personnel is prohibited while wearing civilian attire when in duty status or while in/aboard any ship, craft, aircraft or military vehicle, or within a base or other place under military jurisdiction or while participating in any organized military recreational activity. Either male or female personnel shall attach to or through the ear or nose no other articles.
5. ALCOHOL, TOBACCO AND MEDICATIONS
a. Alcohol. The consumption of alcoholic beverages is strictly prohibited for all patients at SARD, regardless of reason for treatment or treatment program. You may be required to submit to various Breathalyzer examinations upon return from meetings, evening/weekend liberty and at any point during your treatment. Any dance club, nightclub, bar or other establishment whose primary revenue comes from the sale of alcohol is off limits. If you have any doubts about whether an establishment is off limits or not, consult your case manager or the SARD Department Duty Officer (DDO) at the SARD Quarterdeck. Violation of either policy may result in immediate processing as a treatment failure.

b. Tobacco

(1) SARD is a tobacco-free facility. Smoking cessation classes and medical support are available to any patient upon request.
(2) Smoking/use of smokeless tobacco products is permitted only in designated areas and as your schedule permits. Smoking/use of smokeless tobacco products is not permitted in SARD buildings (including your barracks room), government vehicles, during scheduled physical training or those hours intended for treatment. Violation of this policy may result in immediate processing as a treatment failure.
c. Prescription/Non-Prescription Medications. The SARD Medical Division immediately upon check-in must approve all prescription and non-prescription medications to the facility. You must have at least 5 weeks supply of any maintenance medication (i.e. medicine for high blood pressure etc.) prescribed by your doctor. Vitamins and other nutritional supplements not issued by the SARD Medical Division will be confiscated.
6. BARRACKS (Partial Hospitalization and Patients Berthing in Building 500)
a. Personal belongings must be neatly stowed in the lockers provided.
c. There is no place to secure valuables other than the lockers you will be provided. Safeguarding of personal belongings is the responsibility of the owner. Therefore, bringing or purchasing valuables while you are here is strongly discouraged.
d. Clean up of personal living spaces is conducted each day. Cleaning gear must in the cleaning gear lockers, not in your room.
e. Irons and ironing boards are available in the Patient Laundry Rooms located on Floors 2 and 3. The use of irons is restricted to these rooms only.
f. Secure all lights, A/C units and windows prior to leaving your room.
g. Laundry facilities are available in Building 500 free of charge; however, patients are responsible for providing their own laundry detergent and fabric softener. Only patients assigned to that floor will utilize laundry facilities.
h. Females are restricted from male patient berthing areas; males are restricted from female berthing areas. Patient berthing areas include individual barracks rooms, laundry rooms, and lounges.
i. Use of emergency exits/fire escapes is permitted in the event of an emergency or emergency drills only.

7. TELEPHONES AND INCOMING CALLS


a. The use of government telephones for conducting personal business is prohibited. Pay phones are available for local and long-distance personal calls. Personal phone calls may be made as schedule permits during the day.
b. Callers may contact the SARD quarterdeck, Commercial (619) 553-0084 and leave a message. Message will be posted outside the Quarterdeck. Presence at this command will neither be confirmed nor denied to callers due to patient confidentiality.

c. Patients may not use phones located in any office without the presence of a staff member.


8. HEALTH AND COMFORT INSPECTION. All patients receiving treatment within Building 500 are to, without advance notice, a Health and Comfort Inspection, which may include a locker search. Any unauthorized items will be confiscated. Unauthorized items include: alcohol, controlled substance/items, vitamins and medications that have not been cleared by medical, pornography, weapons (including knives), and food and beverages.
10. LEAVE AND LIBERTY. Liberty will normally be granted while in treatment but may be modified depending on your status within that program. Specifically:
a. Outpatient Half-Day Treatment Program. Upon completion of daily treatment, you will be directed to return to your parent command for the remainder of the workday and to return to SARD according to the Daily Patient Schedule.
b. Intensive Outpatient Treatment Program. You will be required to attend community self-help meetings during the evening hours. Number of meetings required will be provided upon arrival for treatment. On "off-evenings" and following meetings, authorized patients will be granted liberty according to the Daily Patient Schedule.
c. Full Day Comprehensive Treatment Program. You will be required to attend community self-help meetings during the evening hours. Number of meetings required will be provided upon arrival for treatment. On "off-evenings" and following meetings, authorized patients will be granted liberty according to the Daily Patient Schedule.

d. Regular Leave. Regular leave will not be granted during treatment. If you wish to take leave post-treatment, you must make all necessary arrangements with your parent command prior to reporting for treatment.


11. FAMILY PROGRAM. If you are married, your family is eligible to participate in the Family Therapy Program conducted during treatment. This program provides information and insight into the disease of alcoholism and drug addiction.
